2019 TAPA Scholarship Information

Before proceeding, review the following eligibility checklist. You must meet the following criteria to be eligible to apply:

          [ ] Current TAPA student member?
          [ ] Currently attending an accredited Texas PA program?
          [ ] Able to submit one set of PA Program transcripts?

Applications will be reviewed and judged on:
          1. Academic record
          2. Community, leadership, and scholarly activities
          3. Future goals as a physician assistant
          4. Applicant evaluation

All parts of the application must be received by January 5, 2019.

Winners will be notified by January 26 , 2019 and announced at the annual business meeting held at the CME meeting in Houston, Texas, February 8-10, 2019.

Electronic Submission Instructions

          1.  Please fill out Scholarship Part 1 & 2 thoroughly. 
          2.  Please send the Scholarship Part 3 Evaluation link to  a mentor, physician assistant, program faculty, physician, or other person who is familiar with
                your accomplishments, community activities, and future goals as a physician assistant.  They will fill out the form ans submit it to TAPA.
          3.  Please send the Scholarship Part 4 - PA Program Director Verification link to your PA program director or their designee.  They will fill out the form
               and submit it to TAPA.
          4.  You must request a copy of your most recent transcript from your PA program (Unofficial transcripts must be signed by PA Program Director).  This
                can be mailed to:

              This must be postmarked by January 5th to be submitted with your application to the judges.  Please make your requests early and confirm with
               your PA program that the transcripts were mailed. 

 

Printed Application Submission Instructions

          1. Download the entire application here.  You must send in all parts of this application together.
          2.  Answer all questions in the application thoroughly.
          3. The applicant evaluation should come from a mentor, physician assistant, program faculty, physician, or other person who is familiar with your
              accomplishments, community activities, and future goals as a physician assistant. The applicant should provide the person completing the evaluation
              with a pre-addressed stamped envelope to mail the form directly to the address below:

          3. Your application package should include the following:
                    [ ] Scholarship application,
                    [ ] PA program verification statement
                    [ ] One copy of your most recent PA program transcript
                        (Unofficial transcripts must be signed by PA Program Director)
                    [ ]  Your evaluation will be submitted by your evaluator. Please follow up with your evaluator to make sure they have submitted the form to TAPA.

          4. All required documentation (submitted by applicant) and applicant evaluation form (submitted by evaluator) should be postmarked
              by January 5, 2019.
